Introduction

Methadone is an Opioid agonist which is indicated for detoxification and maintenance treatment of opioid addiction (heroin or other morphine-like drugs) and also for the treatment of moderate to severe pain not responsive to nonnarcotic analgesics.

Metabolism: Methadone is metabolized in the liver, mainly by N-demethylation and cyclisation, and the metabolites are excreted in the bile and urine. Metabolism is primarily catalyzed by CYP3A4 , although other cytochrome p450 isoenzymes also play a role.

Pharmacodynamics

  • Contra-indications:
Methadone is contraindicated in patients with a known hypersensitivity to methadone hydrochloride or any other ingredient in methadone hydrochloride tablets. Methadone is contraindicated in any situation where opioids are contraindicated such as: patients with respiratory depression (in the absence of resuscitative equipment or in unmonitored settings), and in patients with acute bronchial asthma or hypercarbia. Methadone is contraindicated in any patient who has or is suspected of having a paralytic ileus.
  • Cautions:
Life-threatening Respiratory Depression: Respiratory depression, including fatal cases, have been reported during initiation and conversion of patients to methadone, and even when the drug has been used as recommended and not misused or abused.
Keep methadone hydrochloride (methadone) tablets out of the reach of children. Accidental overdose by a child is a medical emergency and can result in death. If a child accidentally takes methadone hydrochloride (methadone) tablets, get emergency help right away.
  • Side-effects:
